ANSI Ports - Did we ever write the proposal?

Dear Mike McNamara & BTF -

We passed an ANSI port proposal for modules, UDPs, etc. (B09, B36 & B37).

I have copies of Mike's e-mails that show a couple of examples and the
modified BNF but I do not seem to have verbiage that was supposed to be
included in the Verilog LRM. Did we ever write and pass the verbiage? I
have updated the BNF to reflect the passed proposal.

If the verbiage exists, could somebody send it ASAP to Joe Daniels at Seva?
All material was supposed to be to Joe by this morning so your prompt
attention would be appreciated.
