terminal_identifier BNF question

Ted & Steve -

A question came up about the square brackets next to the terminal
identifiers in section A.7.5.2. Should they be bold? Are they trying to
show optional arguments? constant_mintypmax_expression is defined elsewhere
in the BNF without either ()'s or []'s. We need your help on this one.
