Remove the word "unsigned" from Annex B

We purposely reserved the keywords signed and unsigned in the 1364-1995
standard, even though they were not used (OVI had proposed usages in OVI's
Verilog 2.0, which were not accepted for 1364-1995). IMHO, even though we
have not chosen to use the unsigned keyword for 1364-2000, I think it is a
good idea to keep it reserved for possible future standards.
