Though in chapter 12 there is a reference back to chapter 7, so arrays of
modules are covered.

That's far too vague. For instance, an array of regs can be referenced
using a variable index. Surely this is not allowed for an array of
instances. The rules for referencing an instance array should go into the
section on constructing hierarchical names (12.4 I think).

What about changing the definition of genvars from positive to unsigned (or
signed)?
