The standard is actually not silent on this point. See section 19.3.1:

    The text specified for macro text shall not be split across the
    following lexical tokens: Comments Numbers Strings Identifiers Keywords
    Operators

As for my question about +:, I was curious because +: is not listed anywhere
as a single two-character token. Note that section A.8.6 lists operators,
with the implication that each operator is an indivisible unit, but +: and
-: are not included in this list.

These clarifications could go in the LRM, or in an on-line erratum document.
