    All,

    Kurt Baty, Jay Lawrence, Tom Fitzpatrick and myself (Stu Sutherland) were
    assigned the task of coming up with a user-input survey form that could be
    used at SNUG, ICU and possibly other upcoming conferences where there will
    be large numbers of Verilog users. The attached PDF file has the survey we
    have devised. We would like to get feedback from the BTF on this survey
    form during the conference call this Monday, July 28th. Please take a
    moment to review the form prior to the meeting.
