    Dan Jacobi points out that 12 productions in A.8.4, A.8.5 could
    be compressed to 3. I find his versions easier to understand, too.

    In primary (A.8.4, Syntax 4-2), the 4 hierarchical_identifier
    productions can be compressed to --

       hierarchical_identifier {[expression]}
                               [[range_expression]]

    In net_lvalue (A.8.5, Syntax 9-3), the 4 hierarchical_net_identifier
    productions can be compressed to --

       hierarchical_net_identifier {[constant_expression]}
                                   [[constant_range_expression]]

    In variable_lvalue (A.8.5, Syntax 9-3), the 4
    hierarchical_variable_identifier
    productions can be compressed to --

       hierarchical_variable_identifier {[expression]}
