    In 2.5.1 after the paragraph about '_' and
    before the Examples add the following paragraph --

      "An x, z, or ? shall not be used in a decimal
       constant except for a single x, z, or ? preceded
       by a digital base format and optionally succeeded
       by underscore characters, indicating that the
       entire number is x or z."

    and at the end of Example 3 of 2.5.1 add the following
    line --

      16'sd? // the same as 16'sbz
