The draft (2005-d2) fails to mention the interaction between defparams and module instance parameter assignment.
So what happens if a defparam exists for a module and the module is instantiated in another module
that has parameter assignments in the instantiation? Which one overrides?

I would suspect the instance param assignment overrides as the defparam only modifies the local value of the
parameter in the child module. The LRM does not state so explicitly - or maybe I missed it.
